As discussed during the Commission on Higher Education (CHED) and the Department of Health (DOH) Orientation and Meeting with Higher Education Institutions (HEIs) on Masterlisting for Vaccination held on March 30, 2021 at 2:00 PM via Zoom, the Commission would like to request the completion of the following survey form accessible through this link: bit.ly/SurveyHEIasVaccinationSite to determine HEIs, with authority to conduct limited face-to-face, who intend and are willing to partner with respective Local Government Units (LGUs) and serve as vaccination sites.

With more vaccines expected to arrive in the country, the Commission encourages HEIs issued with authority to conduct limited face-to-face classes to offer their facilities as vaccination sites. Stated in a CHED Press Release, “As the country looks forward to receiving the next batch of millions of donated and procured vaccines in the 2nd quarter of the year, we must maximize the use of large alternative venues to be readily available so that the priority groups (essential workers, seniors, the indigents) could be inoculated without further delay”.

Attached is a copy of the slide deck on Chapter VI of the National Deployment and Vaccination Plan on the Implementation of Nationwide Vaccination as presented during the said orientation and meeting, for reference.

Wide dissemination of this Memorandum is desired.
